Athletics Consent Information
Below is a consent agreement that you need to read and understand. When registering your player(s), in a subsequent online step, you will be asked to acknowledge that you have read the following consent agreement, and by submitting your online registration form, for basketball and/ or volleyball, you are executing this agreement and all the terms therein.
2024-2025 Volleyball & Basketball Consent Agreements
I hereby consent to have my child/children participate in the Our Lady of the Lake School Athletics Program, covering the volleyball and / or basketball programs during the 2024-2025 school year.
I understand that this is a competitive program (JV & Varsity) and that not all players are guaranteed to play in every game or play an equal amount of time. This does not pertain to the developmental squads (3/4 grades basketball).
I also understand that the program involves practices and games both home and away and that I will be responsible for transportation for my child to those events.
I will not hold Our Lady of the Lake School or any of its representatives/coaches/administrators responsible for any injury or loss incurred by my child.
I understand that any student who is absent from school or who has served an in-school or out of school suspension, will not attend or participate in an athletics program practice or game that day. The timing and duration of such a restriction is at the sole discretion of the Principal.
I understand that a student must participate in an complete day of school (8:10 a.m. to 2:40 p.m.) for full days or half day if that was the official day of school) in order to participate in a practice or game on such day, and that students are expected to attend a complete day of school the day after a night game/practice regardless of how late they get home or how tired they may be.
I UNDERSTAND THAT I MUST SIGN UP TO WORK THE SNACK BAR AND/OR ADMISSIONS, 50/50 AT SOME HOME GAMES. IF MY “WORK” GAME IS RESCHEDULED, I WILL FOLLOW THROUGH WITH THE DATE OF THE RESCHEDULED GAME. IF I AM UNABLE TO FULFILL MY COMMITMENT AFTER SIGNING UP FOR A DATE, I WILL SWITCH DATES WITH ANOTHER PARENT.
WE AGREE TO SUPPORT THE GUIDELINES FOR OUR LADY OF THE LAKE ATHLETIC ACTIVITIES.
Medical Release & Parent Permission
Below is a Medical Release and Parent Permission agreement that you need to read and understand. When registering your player(s), in a subsequent online step, you will be asked to acknowledge that you have read the following release and permission agreement, and by submitting your online registration form, for basketball and / or volleyball, you are executing this agreement and all the terms therein.
MEDICAL RELEASE AND PARENT PERMISSION AGREEMENT
In consideration of the benefit which my son/daughter will derive from the Our Lady of the Lake School (“OLL”) Athletics program, I hereby release Our Lady of the Lake School, its coaches, staff, and administrators, the Our Lady of the Lake Church, its trustees, teachers, staff, parents, and any other individuals associated with the Athletics program from any and all claims or actions whatsoever in connection with the operation, transportation and supervision of this program’s activity and operations.
I understand that injuries may occur in interscholastic sports training and competition. I give permission for my son/daughter whose name appears in the online registration form I have or will have submitted to participate in the Athletics program at OLL. In the event of an emergency or in case I cannot be reached, I give permission for the coaches, the nurse and/or assistant coaches to provide first aid treatment and to act on my behalf to approve emergency transportation and/ or medical treatment.
